Initial SAP Evaluation
The initial SAP evaluation is the federally required first step in the DOT Return-to-Duty process. Dr. Collins conducts a comprehensive clinical assessment to review your DOT violation, substance use history, and work background. Based on this evaluation, he will issue a formal recommendation for education or treatment.

Follow-Up SAP Evaluation
After completing your recommended education or treatment program, you will return to Dr. Collins for your follow-up evaluation. This appointment determines whether you have successfully complied with the SAP's recommendations and whether you are eligible to take your Return-to-Duty drug or alcohol test.
