What Happens During Your SAP Evaluation
1. Welcome & Identity Verification
5 minutesDr. Collins will greet you warmly and verify your identity using a government-issued photo ID (driver's license or passport). If your evaluation is virtual, you will hold your ID up to the camera. This step is required by DOT regulations to confirm you are the correct individual. There is nothing intimidating about this β it is simply a standard federal requirement.
2. Review of Your DOT Violation
10β15 minutesDr. Collins will review the details of your specific DOT drug or alcohol violation. This includes the type of violation (positive test, refusal, etc.), the substance involved, the date and circumstances, and any documentation from your employer or testing facility. He is not here to judge you β he simply needs to understand the full picture to make an accurate recommendation.
3. Clinical Interview & Substance Use History
20β30 minutesThis is the heart of the evaluation. Dr. Collins will ask you a series of structured clinical questions about your substance use history. Questions may include: How often do you use alcohol or drugs? Have you ever tried to cut back? Has your use ever caused problems at work, home, or with the law? Have you ever been in treatment before? Be honest β Dr. Collins is not trying to catch you out. He needs accurate information to make the right recommendation for you. Honesty actually leads to better, faster outcomes.
4. Standardized Screening Tools
10 minutesDr. Collins will administer standardized, federally recognized screening questionnaires. These typically include the DAST-10 (Drug Abuse Screening Test) and the AUDIT (Alcohol Use Disorders Identification Test). These are short, straightforward questionnaires β not trick questions. 5. Review of Work & Personal History

Will my employer find out what I said during the evaluation?
Dr. Collins is required by DOT regulations to report certain information to your employer's Designated Employer Representative (DER) and to the FMCSA Clearinghouse β specifically, that you have completed the evaluation and what your recommendation is.
